An Angus Health and Social Care Partnership survey
Angus Health and Social Care Partnership would like to invite you to participate in a survey for Angus residents (adults aged 16+ years only).
Your feedback will help:
- Find out what your needs and views are in relation to health promotion and prevention of chronic illness
- Find out what your expectations and suggestions are in relation to promoting health and preventing chronic illness in Angus
- Find out what opportunities you currently access and/or would like to access in the future
- Find out potential opportunities for community development and innovative projects in Angus.
Your information will help inform Angus strategy, locality improvement plans and potential change projects.
The survey is confidential. You will therefore not be identified in the survey report.
The deadline for completing the survey is 31 January 2019.
